The importance of accuracy in patient names

I recently came across a case where a simple typo in a patient’s name led to a mix-up with their medication schedule. It’s a good reminder of how crucial it’s for us to double-check every detail, even the seemingly obvious ones. Has anyone else experienced something similar that made you rethink your data entry practices?

I once had a similar situation where a small name error almost led to a mix-up at discharge. Now, I always run a final check with the patient’s ID against their chart before closing. It’s amazing how often that catches minor mistakes.
